The APCC Program expert reference panels, in consultation with Diabetes Australia and Heart Foundation Australia, have confirmed new diabetes and CHD measures for the Program.

The evidence behind the new measures will be introduced to practices via two back to back plenary sessions in Perth, Fri 28 Nov 2008.
